The inner (fascial) layer is stretched during pregnancy, but unlike skin, does not have the elasticity to contract. Surgically tightening the inner core layer to a normal tone is important in supporting the back and restoring a flat tummy.
After pregnancy and breastfeeding, the breasts stay enlarged for months, and the skin stretches. When milk production stops, the breast glands shrink. The combination of stretched skin and loss of volume causes the breast to appear flat and lose the round, pre-baby body shape. It is very important to design the incisions so that the final scars can be hidden. Ideally, we are trying to create an hourglass shape with waist definition being the narrowest part. In many mommy makeover patients, prior to surgery, their torsos have a squareness, appearing equally long as wide. After, the waist is defined, and by flattening the tummy the torso or abdomen appears to have a beautiful proportion.
When the tummy tuck is designed, the excess skin is marked for removal. But more important is leaving enough skin so that the final scar has no tension and can sit very low on the body, just at the pubic edge. If there is tightness on the scar it will widen or contract rather than being hidden. There should be a central “section” of the tummy with a natural depression around the belly button.
